I, [Participant's Name], hereby acknowledge that I am voluntarily participating in the workshop experience provided by OLPHACTORY CANDLES, POUR’d: a sensory experience and POUR'd by U. I understand that there are inherent risks associated with this activity, including but not limited to burns, allergic reactions, or other potential hazards.
In consideration for being permitted to participate in this activity and/or purchasing tickets, I hereby waive, release, and discharge OLPHACTORY CANDLES, POUR’d: a sensory experience and POUR'd by U, its owners, employees, and affiliates from any and all liability for any harm, injury, or damages that I may sustain as a result of my participation in the sensory experience provided by OLPHACTORY CANDLES, POUR’d: a sensory experience and POUR'd by U.
I understand and acknowledge that this waiver of liability includes, but is not limited to, any claims based on negligence, action, or inaction of OLPHACTORY CANDLES, POUR’d: a sensory experience and POUR'd by U or its representatives.
I also agree to indemnify and hold harmless OLPHACTORY CANDLES, POUR’d: a sensory experience and POUR'd by U, its owners, employees, and affiliates from any claims or liabilities arising from my participation in this activity.
